Adding custom HTML is as simple as adding any other element.
All you need to do is:
1- Enter your form in the form builder.
2- Drag the 'HTML' element from the upper toolbar on to the design area (  ).
3- Once the element is selected, go to its 'element settings' on the right and type your HTML string (or paste it) in the source property.
