Go to the the "Data Center".

Press on the 'Delete Form' icon in the relevant form row.
(For example the screenshot below shows an attempt to delete the 'applications form').

A confirmation window will appear twice, making sure that the user does not delete the form by mistake.

|

Press "ok" to delete the form.
