Drag the Multi line element into the form design area.
Notice that this element is made out of 2 containers (one inside the other) and 2 buttons (add and remove).

Resize the exterior container (to fit the elements you want to place inside it) and the interior one accordingly, and start dragging the fields into the container.
All elements inside the container will be duplicated, so if you do not wish to have the labels duplicated just leave them outside.
Place the add button outside the container in alignment with the remove button.

Change the interior border to 'none'. (** this is optional-for design purposes only)

Change the background color of the exterior container. (** this is optional-for design purposes only)

Once you enter the form all you will see is one empty row - just like you designed it in the form creator.

When you press on the 'Add' button a scroll will be installed in the container and another empty row will be added. Now you can scroll down and fill in the second row. Pressing on submit will send the form.
